Enter Johnny Cage, a washed-up but still famous Hollywood martial-arts star whose career has slid into self-parody. He is introduced on a chaotic film set, where his inflated ego, stunt-man bravado, and floundering franchise reflect a man clinging to former glory. A mysterious incident, tied to his latent connection to Earthrealm's champions, draws the attention of Raiden, the thunder god, and Sonya. When Raiden appears to Cage, he dismisses him as some elaborate studio gag, but a brief display of divine power and an attack by Outworld assassins make the stakes impossible to ignore.

Raiden gathers Johnny Cage, Cole Young, Liu Kang, Kung Lao (in a spiritual/Netherrealm-linked capacity following his prior fate), Sonya, and Jax at his temple, making it clear that the time for an official Mortal Kombat tournament has arrived. If Earthrealm loses, Shao Kahn gains the legal right, under ancient cosmic rules, to invade and conquer Earth outright. Training sequences at the temple emphasize each fighter's attempt to unlock or refine their arcana, their unique inner power, as Raiden and Liu Kang push them to overcome fear, ego, guilt, and trauma.

Meanwhile, in Outworld, Shao Kahn prepares his own champions. He commands Shang Tsung, the ice-cold assassin Bi-Han/Sub-Zero, and the resurrected, vicious Revenant Kano to sow chaos and demoralize Earthrealm before the tournament formally begins. He keeps Edenian queen Sindel close, using mind-bending sorcery and political pressure, while eyeing Kitana as a potential weapon: her skills with her bladed fans and her status as Edenian royalty make her both an asset and a threat. At Shao Kahn's side is Jade, a deadly Edenian assassin torn between her duty to the throne and a lingering sense of loyalty to Kitana.

In a key early middle-act sequence, Raiden secretly reaches out to Kitana, hinting that her true heritage ties her to Edenia's lost freedom rather than Shao Kahn's empire. From here, the story alternates between character-driven training and bonding on Earthrealm's side and ominous political maneuvering and brutality in Outworld. Johnny's arc is central: beneath his narcissism lies a man afraid he is a fraud, terrified that real combat will expose him. Liu Kang grapples with the pressure of being a de facto champion and successor to an ancient tradition, while Cole must redefine his place now that his initial prophecy has been fulfilled. Sonya and Jax confront the human toll of their new responsibilities, as more strange deaths and disappearances hint that Outworld is already testing the boundaries of the rules.

When the tournament's location is finally announced, spanning arenas across Outworld and neutral spaces between realms, Raiden leads his chosen fighters through portals into alien landscapes: decaying Outworld coliseums, spectral Netherrealm gateways, and a mysterious Edenian ruin that bears scars from Shao Kahn's conquest. Along the way, they face ambushes by Quan Chi, a sinister Netherrealm sorcerer whose manipulations crosscut the loyalties of Shang Tsung, Bi-Han, and the revenant warriors. These confrontations deliver brutal, game-inspired fight sequences, but the film stops short of its final tournament rounds at this point in the synopsis.

The central conflict solidifies: Earthrealm's flawed but evolving team must overcome their personal baggage, accept Johnny Cage as an unlikely linchpin, and navigate shifting alliances with Kitana and Jade to prevent Shao Kahn from winning Mortal Kombat and claiming Earthrealm.
