Makima is the physical manifestation of the Control Devil, a primordial entity born from humanity's collective fear of being dominated. She entered the world in ancient times and has survived through various incarnations by consuming previous versions of herself or waiting for new vessels to emerge. Her existence predates modern civilization, allowing her to observe human history and understand the deepest fears that drive society.

She adopted the persona of a calm, composed government employee to infiltrate the Tokyo Public Safety Bureau. By rising through the ranks, she secured leadership of Special Division 4, the team dedicated to hunting the Chainsaw Devil. This dynamic shifts dramatically when Denji discovers her plan to kill him and consume the power of the Chainsaw Devil.

The turning point arrives as she systematically breaks Denji's spirit by torturing his friends, including Power and Aki Hayakawa. She reveals her sadistic nature by forcing Aki to kill his family and then killing Aki himself to test Denji's resolve. The conflict culminates in a brutal battle where Denji, having fully accepted his identity as the Chainsaw Man, fights back against her control.
