Dungeons are procedurally generated with randomized layouts, monster compositions, and objectives, offering targeted loot farming through a Codex of Power that extracts legendary aspects.

    Each class has a distinct resource system and skill tree that allows for varied builds. The Druid, for example, can shapeshift into werebear or werewolf forms and call upon nature magic, while the Necromancer raises skeletal armies and uses corpse-based abilities. At level 50, the Paragon Board provides a grid of tiles for passive bonuses and glyph sockets, enabling deep customization. The game also features a robust crafting system for upgrading gear, imprinting aspects, and socketing gems.

    The shared-world structure supports up to four-player parties for dungeons and world content. PvP occurs in the Fields of Hatred, where players can collect Seeds of Hatred and extract them at altars while fending off other players. Seasonal content, starting with Season of the Malignant in July 2023, introduces new mechanics, a battle pass with cosmetic rewards, and a seasonal journey that resets character progression.     Story

    The story begins with the summoning of Lilith, daughter of Mephisto, to Sanctuary by a cult seeking to free humanity from the influence of both Heaven and Hell. The player character, the Wanderer, escapes the destruction of their village and meets the Horadric scholar Lorath Nahr, who explains the return of the Prime Evils. Together, they track Lilith across the five regions, uncovering her plan to corrupt the world and raise an army of demons.

    Key revelations include the return of Inarius, the angel who created Sanctuary with Lilith, who has been imprisoned in Hell. The Wanderer collects fragments of a worldstone to confront Lilith, eventually facing her in the Necropolis of the Firstborn. The campaign culminates in a battle where Lilith is defeated, but her death unleashes Mephisto's essence, setting up the expansion Vessel of Hatred. The narrative is delivered through extensive cinematics, environmental storytelling, and side quests that explore the history of Sanctuary and the fates of its inhabitants.
