Rosie Dunne and Alex Stewart have been best friends since childhood, sharing every milestone and secret. On the night of Rosie's 18th birthday, a misunderstanding sets off a chain of events that will define their futures. Alex had planned to confess his feelings, but Rosie ends up at the party with popular classmate Greg, while Alex is distracted by Bethany, a girl he thinks is out of his league. That same night, Rosie discovers she is pregnant with Greg's child, dashing her plans to attend university in Boston with Alex.

Rosie decides to raise her daughter Katie alone, while Alex moves to Boston to study medicine, encouraged by his father. He begins a relationship with Bethany, and when Rosie visits him, she learns Bethany is pregnant with Alex's child. A heated argument ensues, with Alex cruelly pointing out that his child will have two parents while Rosie is a single mother. This rift drives them apart for years.

Rosie marries Greg, but soon discovers his infidelity and divorces him. She builds a career in hotel management while raising Katie. Alex, after his relationship with Bethany ends, remains distant. A letter from Rosie confessing her true feelings and revealing her pregnancy is hidden by Greg, never reaching Alex. Over the years, they maintain contact through phone calls and chance meetings, always circling back to each other but never quite bridging the gap. Alex becomes a successful doctor and gets engaged to Bethany, while Rosie continues to wonder what might have been.
