Three years after a genetically engineered measles virus, developed as a cancer cure, mutates and wipes out 99% of humanity, Dr. Robert Neville lives alone in a fortified Washington Square Park townhouse. The virus killed 90% of those infected and turned the remaining 9% into Darkseekers, nocturnal, hyper-aggressive creatures with superhuman strength. Only 1% of the population, like Neville, proved naturally immune.
Neville's days follow a strict routine: he hunts deer through the overgrown streets of Manhattan, broadcasts AM radio messages hoping for survivors, and conducts dangerous medical experiments on captured Darkseekers, using his own blood to search for a cure. His only companion is his German Shepherd, Sam, a loyal military-trained dog who provides emotional support and tactical backup during hunts. At night, he barricades himself in his lab as the creatures prowl outside.
The isolation has taken a toll. Neville talks to mannequins he has arranged in a video store, giving them names and scripted conversations. He struggles with survivor's guilt over losing his wife Zoe and daughter Marley in the initial evacuation chaos. The central conflict is twofold: Neville must survive the nightly attacks of the Darkseekers, led by an intelligent Alpha Male with heightened cunning, while racing against time to develop a vaccine before his own psychological stability crumbles.
A major turning point occurs when Neville traps a female Darkseeker and tests his experimental cure, showing initial promising results. However, his isolation is shattered when Sam is infected during a confrontation with the creatures, forcing Neville to make an impossible choice. Shortly after, Neville encounters Anna Montez and a young boy named Ethan, who respond to his radio broadcasts, but their arrival also brings the Alpha Male and his horde directly to Neville's doorstep, setting up the film's climactic confrontation.
