Fallout 76 drops you into the Appalachian wasteland in 2102, 25 years after the bombs fell. You emerge from Vault 76, one of the few intact shelters, tasked with repopulating the region. But instead of friendly faces, you find a land ravaged by the Scorched Plague, a mysterious disease that turned the local population into feral creatures. Bethesda Game Studios built this as their first multiplayer title, a bold shift from the single-player RPGs that defined the series. Developed alongside BattleCry Studios, it runs on a modified Creation Engine optimized for online play. The game launched on November 14, 2018, for PlayStation 4, Windows, and Xbox One, with later upgrades for PlayStation 5 and Xbox Series X|S.

From the start, Fallout 76 courted controversy. The initial release was riddled with bugs, lacked human NPCs, and felt aimless to many players. Fallout 76 is an online survival RPG where you explore West Virginia's open world, scavenge for resources, and fight mutated creatures. The combat uses real-time V.A.T.S., a change from previous games where it paused the action. You build and manage CAMPs, mobile bases you can place almost anywhere. The S.P.E.C.I.A.L. system returns, but perks are now cards you collect and swap freely. Public events, daily quests, and expeditions like The Pitt offer repeatable challenges. Up to 24 players share a server, but you can team up in groups of four. PvP is opt-in, with a bounty system for those who attack others.

A subscription called Fallout 1st gives private servers and a scrapbox with unlimited storage. The game also features seasonal events like Fasnacht Day and Mothman Equinox. The Wastelanders update introduced two factions: Settlers and Raiders, both after a treasure of gold bullion. Steel Dawn brought the Brotherhood of Steel, and Steel Reign continued their story. The Pitt expansion sent players to irradiated Pittsburgh for expeditions. Atlantic City added a new gambling hub. The Skyline Valley update in 2024 introduced a rogue Enclave AI.
