Nielsen's career is defined by iconic roles in some of the biggest films of the past two decades. While she hasn't collected many individual trophies, her work in Gladiator earned her a SAG Award as part of the cast and a Blockbuster Entertainment Award. She was also honored by her home country with a Danish Film Award for the same performance. In 2021, the Danish American Society named her Danish-American of the Year for her contributions to film and culture.

AwardsNielsen won a Blockbuster Entertainment Award for Favorite Supporting Actress in a Action/Sci-Fi film for Gladiator in 2001. That same year, she received a Danish Film Award (Robert) for Best Actress in a Leading Role for the same film. She also shared in the Screen Actors Guild Award for Outstanding Performance by a Cast in a Motion Picture as part of the Gladiator ensemble.
